Driving Patient Care Success: Robin’s Journey with i2iTracks

Munising Memorial Hospital adopted i2iTracks in 2022, and since then, Robin has transformed not only her own expertise with the platform but also the entire organization’s buy-in and appreciation for its impact.
“i2i has been one of the biggest assets to our clinic! We love it, and both our front and back office teams use it daily,” says Robin.
Tackling Annual Wellness Visits with the Recall Function
One of the biggest improvements Robin spearheaded was optimizing the Recall Function to bring patients back for annual wellness exams and well-child visits.
“At Bay Care, we had no way in our EMR to recall patients. We discovered we were missing a significant number of annual wellness visits, and the root cause was the lack of a recall system,” Robin explained.
Working closely with her PHC, Robin implemented i2iTracks’ Recall Functionality—and the results have been remarkable.
“Since launching this process over a year and a half ago, we’ve significantly increased our annual and well-child visits,” she shares.
The Recall Function now operates through a structured workflow:
- Ready Queue – Patients due for their annual visit are identified.
- Waiting Queue – After the first outreach letter, patients have 60 days to respond.
- Second Outreach – If no appointment is scheduled, patients return to the Ready Queue for a second letter.
- Failed Queue – After two unsuccessful outreach attempts, the front office proactively follows up with phone calls.
“This process is now fully hardwired into our workflow, and our front office has taken full ownership of managing recalls,” Robin proudly states.
Enhancing Care Coordination with Patient Visit Summaries
Robin’s team also introduced the Patient Visit Summary as a key tool for chart prep and seamless provider hand-offs.
Now, after every visit:
- The provider documents the patient’s follow-up needs on the Visit Summary.
- The patient brings it to Registration/Checkout, ensuring their next appointment is scheduled on the spot.
“This has made our care team more proactive and prepared for visits, while also reducing the number of patients lost to follow-up. I love this product!!” Robin exclaims.
