With our market-driven and future-focused strategy we are identifying solution growth opportunities to generate value for both i2i customers and i2i. We are bringing providers and health plans together to collaborate and design a better set of value-based care solutions that helps consumers, members, and patients to realize higher quality, better outcomes and improved satisfaction. In i2iTracks the Value-based Care (VBC) Tracker, planned for a release mid-year, will put tools and dashboards in providers’ hands to enable management and tracking of quality incentive programs, attributed patients, measures and incentive dollars. In partnership with health plans, it will integrate claims and clinical data to provide unique insights into quality performance. Also, keep your eyes open for our new product, iQlarityTM, scheduled for release this summer! iQlarityTM is our new web-based product for health plans that combines real-time, electronic health record data with health plan administrative data to power big data analytics that drives improved quality outcomes. Simplifying and enhancing care quality across the continuum is what iQlarityTM and the Value-based Care Tracker will deliver for health plans and providers.
We are also excited about other new i2iTracks features that will be available during 2018. i2iTracks care management and patient engagement capabilities will be expanding with expanded risk stratification capabilities, care coordination, chronic care management, and clinical pathway reporting. We will be partnering with MeU Care to integrate patient engagement capabilities into i2iTracks, enhancing your patient communications and your patients’ experiences. Around mid-year you can also look forward to new toolkits: Caries Risk Assessment, PRAPARE, Social Determinants of Health, and Medication Assisted Therapy.
